Complete Eye Examinations
Many health problems can be diagnosed in their early stages through a comprehensive medical eye examination. Some eye disorders such as glaucoma begin without any symptoms at all; and sometimes even minor symptoms such as blurry or cloudy vision, dry eyes or eye discoloration can signal a physical illness not related to the eye, such as diabetes or high blood pressure. We urge you to keep your routine eye exam appointments and to maintain your overall health.A thorough eye exam requires evaluation of the "back of the eye" which will require us to dilate your eyes. The necessary drops may cause temporary blurred vision or light-sensitivity; therefore, we ask that you bring a friend or family member to drive you home. Your First Visit
So that Florida Eye Health can best meet your needs, we ask you to complete our registration and health history forms during your first visit. It is
important for us to know about all medications you are taking and about any allergies you may have. Previous medical records concerning your eye health are also helpful. Please remember to
bring your eyeglasses to each appointment. Filing insurance claims can be confusing. Please bring your insurance cards so that we can make copies of them. . Concerned Care for Senior Citizens
Many eye disorders are caused by the effects of aging, so we are especially sensitive to the specialized needs of our older patients. We are happy to schedule
convenient appointment times for our senior patients and arrange transportation if necessary. Patient Education
Proper eye care can prevent many eye conditions and early detection can protect
your vision. We continually advise our patients on topics related to eye care and treatment of eye disorders through office newsletters, free brochures and literature, private lectures for local clubs and
organizations, and complimentary community seminars and eye screenings. Affordable Fees
Our patients are our priority. We want to provide you with the high level of eye care you
deserve. Therefore, we appreciate that payment is received for all office services at the time they are rendered. Our financial counselor is available to guide and assist you in various methods of
payment. Our billing department is happy to answer inquiries during regular business hours, 8 a.m. to 5 p.m., Monday through Friday. Medicare Recipients Welcome
We
accept Medicare assignment on all office examinations and procedures and reduce our fees to the Medicare allowable. Of this amount, Medicare pays 80% and the patient is responsible for the 20% co-payment
plus any deductible not met for the year. Convenient Appointments and Emergency Care
Our office hours and locations are designed to meet your needs. Locations throughout Lee, Charlotte, and Collier County -- Fort Myers, Cape Coral, Punta Gorda, Lehigh Acres, and Naples -- make it convenient for you and your family members. Office appointments are available Monday through Friday between 8 a.m. and 5 p.m.You can be confident of our response during an emergency -- 24 hours a day, including weekends and holidays. Simply call our office at any time and our answering service will promptly
relay your concern to Dr. Frantz. He will contact you as soon as possible to make arrangements for emergency treatment if necessary. If you experience trauma to the eye or to the area surrounding
the eye, or if you believe you have an extreme eye emergency, go to the nearest hospital emergency room and request the hospital staff to call Dr. Frantz. Doctors on hand will begin proper diagnosis and
treatment until Dr. Frantz arrives. On occasions when Dr. Frantz is out of town, emergencies will be handled by another qualified, board-certified ophthalmologist.
